Typhoid fever

Extensively drug-resistant (XDR) typhoid fever outbreak in Sindh province, Pakistan, ongoing since it began in Hyderabad district in November 2016. WHO reported 5,274 XDR cases out of 8,188 total typhoid fever cases between 1 November 2016 and 9 December 2018 (69% in Karachi, 27% in Hyderabad district, 4% elsewhere in Sindh). Six travel-associated XDR cases were reported abroad (UK, US) by October 2018. More recent reporting (Coalition against Typhoid, July 2025, citing WHO Pakistan Country Office and Pakistan's Expanded Programme on Immunization) points to a sustained decline in confirmed typhoid cases since 2023, following nationwide typhoid conjugate vaccine campaigns (over 30 million children vaccinated since 2019) and an expansion of sentinel surveillance sites from 9 to 22. Current surveillance snapshot: NIH Pakistan's weekly Integrated Disease Surveillance and Response (IDSR) Public Health Bulletin (Vol. 5, Week 27, 5-11 July 2026, published 17 July 2026) recorded 1,776 suspected typhoid cases nationally that week (844 Sindh, 620 Khyber Pakhtunkhwa, 211 Balochistan), with laboratory confirmation of 5/146 tested in Sindh, 15/77 in Balochistan and 3/204 in Punjab; this bulletin does not break out XDR status separately from typhoid overall. The same July 2026 issue reported an NIH-UKHSA capacity-building initiative to standardize healthcare-associated infection surveillance in Balochistan via DHIS-2.
